1. Regarding student ID cards (identification documents)
Your student ID is the most important document that proves your student status. Always carry it with you and be sure to follow the instructions printed on the back. Please handle it with care, as you will be using it for four years.
Please be sure to present your student ID card when taking exams or other examinations.

2. Reissuance of student ID card
If your student ID card is lost, stolen, or damaged, please report it immediately to the Student Affairs Office (Student Welfare Section) and apply for a replacement. To apply, purchase a replacement application form from the automatic certificate issuing machine located on each campus and submit it to the Student Affairs Office (Student Welfare Section). You can expect to receive your replacement card in about a week. The replacement fee is 2,000 yen.
If you wish to change your photo, please bring one new photo (taken within the last three months, showing your upper body, front view, without a hat, against a plain background, 4cm high x 3cm wide). The fee for changing a photo or reissuing a damaged photo is 1,000 yen. Replacements due to magnetic strip malfunctions or name changes are free of charge. (Please inquire at the counter for details.)
3. Issuance of temporary student ID card
If you are asked to present your student ID during an exam or other examination and do not have it with you, please obtain a temporary student ID from the automatic certificate issuing machine located on each campus. (500 yen; valid only on the day of issuance)
