Regarding the issuance of various certificates
(For undergraduate graduates)

Certificate type, fee, and processing time

The types of certificates, the issuance fee per certificate, and the estimated number of days from the time of application to issuance after the university has completed the application process are as follows.

  • This document lists the faculty and department of graduation, the date of enrollment, and the date of graduation.
  • For those who graduated in or after March 1997 (Heisei 9), their degree is also listed.

Academic transcript (undergraduate) 200 yen Same day to 1 week This shows the names of the subjects for which you earned credits, the number of credits earned, and the scores you received. (Subjects for which you did not earn credits are not listed.)

  • Please provide your name in Roman letters, the recipient, and the purpose of use.
  • The (English) graduation certificate will also include the name of the degree.

Certificate of academic ability
(Former teacher's license basic qualification/credit acquisition certificate)

  • This is a certificate required to obtain or apply for a teaching license. (The name was changed in 2009.)
  • One copy is required for each type of license.
  • Please let us know whether you require proof under the new or old Teacher Licensing Act.
Basic Qualifications and Credit Acquisition Certificates 300 yen Approximately 1 to 10 days
  • This is a certificate for qualifications other than teaching that can be obtained at this university.
  • The types of certificates that can be issued vary depending on the faculty, department, and year of enrollment.
  • Please let us know the type of qualification you have (e.g., museum curator, social education specialist, librarian, school librarian teacher, Japanese language teacher). We will issue a certificate for each type of qualification.
Certificate of Units for Qualification as a Chief Electrical Engineer 400 yen Approximately 1 to 10 days
Certificate of Credits Acquired for Designated Subjects in the First-Class Architect Examination / Graduation Certificate 400 yen Approximately 1 to 10 days

Identity Verification 400 yen Approximately 1 to 10 days This certificate is for use by those who have obtained a teaching license from this university when taking the teacher employment examination. (It will only be issued upon request from the Board of Education, etc.)

How to apply for a certificate

Applications for certificates are accepted in two ways: (1) in person at the counter, and (2) by mail. Please note that even in urgent cases, applications cannot be made by phone, fax, or email due to identity verification and payment processing requirements.

(1) When applying at the counter
Required documents
Please submit the following required documents to the reception desk.
①Application form
Please pay the certificate fee and print out the application form at the "Automatic Certificate Issuing Machine" located on each campus.
Afterward, please fill in the required information such as your name and graduation year on the "Application Form" and submit it to the counter.
② Identification card
For identity verification purposes, you will be required to present a government-issued identification document (such as a driver's license, health insurance card, My Number card, or passport).

*Please note that the only certificates that can be issued at the Gobashi Campus are graduation certificates (in Japanese and English) and academic transcripts (in Japanese and English) for undergraduate graduates from March 1997 (Heisei 9) onwards. For graduates from March 1996 (Heisei 8) or earlier, and for certificates other than graduation certificates and academic transcripts, applications and issuance will be handled at the Tsuchitoi Campus.

Precautions
  • Neither campus has its own parking facilities. Please use public transportation or nearby parking lots.
  • If you wish to receive a certificate by mail that cannot be issued on the same day, please bring a self-addressed stamped envelope.
(2) If applying by mail
Required documents
Please send the following four items in an envelope.
① Download the "Certificate Issuance Application Form" or any other form of paper with the following required information filled in.

*If you are unable to download the form, please provide the following required information on a separate sheet of paper.

  • Years of enrollment and graduation
  • Name of graduating faculty/department
  • Name (phonetic spelling)
  • Student ID number (please fill in if you remember it)
  • Date of birth (please write in Western calendar year)
  • Return address and postal code
  • Please provide a contact phone number (such as a mobile number where you can be reached during the day).
  • Required types and number of certificates
  • Whether or not strict sealing (invalid if opened) is required/not required
② A copy of an identification document issued by a public institution (driver's license, health insurance card, My Number card, etc.)
The copy of your identification document that you have sent us will not be used for any purpose other than verifying your identity when issuing the certificate. After verification, we will dispose of it responsibly.
③ Certificate issuance fee and return postage
  • Please enclose stamps for the certificate fee and return postage (see below). (We cannot accept cash payments as it is not possible to send cash by regular mail.) Please enclose the stamps for the certificate fee and the stamps for the return postage separately.

Please write the recipient's address and name on a standard/long envelope (12cm x 23.5cm). If you are applying for a large number of documents, please use a non-standard envelope. Also, if the certificate is in English and requires sealing, please use a non-standard envelope.

If you wish to have someone apply for and receive the documents on your behalf, the following documents will also be required for both in-person and mail-in applications.

  1. Power of attorney (PDF:51KB)
    (The format is optional, but the applicant's signature and seal are required.)
  2. A copy of the applicant's driver's license, health insurance card, or My Number Card.
  3. The agent (authorized person) must present one of the following: driver's license, health insurance card, or My Number Card (if applying by mail, please enclose a copy).
